Transitions for Women

Empire Health Foundation funds Transitions' new initiative-Bridge the Gap to help low-income and homeless women pay for health care

Bridge the Gap, a new program available to homeless and low-income women and their children who receive services at Miryam's House, Transitional Living Center, the Women's Hearth and New Leaf Bakery will help pay for medical and dental services (up to $500 per woman or child).  The new program is funded through a grant from Empire Health Services, which is the private foundation that was formed as a result of the 2008 sale of Empire Health Services (Deaconess and Valley Hospitals)
